Liability Agreement: In consideration of being permitted by The Clay Lounge to participate in an in-studio class, I hereby waive, release and discharge any and all claims for damages or personal injury, death, or property damage which I may have or which may hereafter accrue as a result of my participation in said activity. This release is intended to discharge, in advance, The Clay Lounge, its officers, directors, members and managers, and all other members of its studio staff whether contracted or employed, from and against any and all liability arising out of or connected to in any way with any participation in said activity. I understand that the activity that I am participating in may be of a hazardous nature and/or include physical and/or strenuous activity. Knowing the risks involved, nevertheless, I have voluntarily applied to participate in said activity, and I hereby agree to assume any and all risks of injury or death and to release and hold harmless The Clay Lounge, its officers, directors, members and managers, and all other members of its studio staff whether contracted or employed, who might otherwise be liable to me.

Illness/behavior: I waive all rights to any legal action against The Clay Lounge in the event of injury, illness, damages, loss of personal property not caused by or resulting from the negligence of the owner or attendees in the studio. Preventative measures will be practiced to reduce the spread illness by refusing admission to anyone appearing to not be feeling well. I understand that The Clay Lounge reserves the right to dismiss anyone from this class for reasons such as, but not limited to, signs of illness, disruptive or unsafe behavior, behavior unsuitable for the comfort of the classroom environment, or poor performance at the sole discretion of The Clay Lounge.

Photo Release: I give permission for my photo to be taken during in-studio classes. I understand that the photos taken at The Clay Lounge are for the sole purpose of documenting the various experiences offered and may be used on social media and website.
